Fixing details are important

In this case study, our engineers encountered a very poor fixing detail. Given that this is a specialised field designed to ensure safety and prevent injury, it is surprising that such issues can occur. This should not happen, and it is imperative to follow proper procedures to prevent it.

Here are some recommendations to ensure proper installation:

1. Use a sign-off sheet upon completion of the installation.

2. Have installers take a picture of each fixing as they install it.

3. Manage from the source by employing only trained and competent installers.

There is no excuse for the shortcomings in this example.
